ONLINE CHECK IN ONLY

On-line check in: Mon-Thurs, Aug 8-11, cut off time Aug 11 at noon.

CHECK-IN PROCEDURES

ON-LINE – Local Teams:

Please complete the following steps to submit your roster for on-line check-in:

  1. Prepare your official Affinity tournament photo roster – instructions can be found at Wasatchsoccer.org
  2. Save a copy of your Affinity roster as a PDF file
  3. Upload your completed roster and any guest player forms (if applicable) to GotSport.
  4. Rosters must be received by Wednesday, August 10th  at noon.
  5. Once your roster has been approved you will need to print 6 copies and bring one to each game.  

ON-LINE – Out of State Teams:

ALL OUT OF STATE TEAMS ARE REQUIRED TO DO ONLINE CHECK IN.

Please complete the following steps to submit your roster for on-line check-in:

  1. Prepare your official state association roster
  2. Prepare a GotSport photo roster – instructions can be found at Wasatchsoccer.org
  3. Upload your official state roster, GotSport photo roster, a copy of all player cards and any guest player forms (if applicable) to GotSport.
  4. Rosters must be received by Thursday, August 11th  at noon.
  5. Once your roster has been approved you will need to print 6 copies and bring one to each game.  

IN-PERSON:

Check-in will be Friday, August 12, from 5-7 pm at the Wasatch Training Center in Layton (976 N Marshall Way, Layton, Utah 84041).

IN STATE (Utah) teams will need the following:

  • 6 copies of an Official Affinity tournament photo rosters (with photos for players AND coaches)
  • Guest players must be included on the photo roster with picture.  Each guest player must have a guest player form signed by the releasing coach and parent
  • Player cards for guest players who are considered tournament-only players

OUT OF STATE TEAMS REQUIRED TO DO ONLINE CHECK IN.

PLEASE NOTE:

  • Teams failing to meet Check-In requirements will not be eligible to play.

​Although not required at Check-In (according to a new UYSA policy) teams should have the following documentation at the field for every game:

  • A medical release form OR UYSA Participant Registration Form with medical consent for EACH player, completed and signed (notary NOT required)
  • If a player’s age is questioned, a Birth Certificate must be provided before the next game (for out of state teams, a player card will constitute proof of age).
